Welcome back!  It's time to start getting the Family Directory together.
A few things:

 

1)      We need a response from you regardless of whether you would like
to be included or not.

2)      If you would like to be included, YOU need to give us the
information.  Due to privacy reasons, we publish ONLY the information
that you provide to us in writing.  

3)      You can return this form to one of your children's classroom
teachers, (i.e. if you have more than one child, you only need to do
this once and send it in with one of them).

DIRECTORY ORDER FORM

Copies of the directory can be purchased for $5.00 each.  There is no
fee to be listed in the directory.  If you would like to purchase one,
please mark the number of copies below and include your money in an
envelope with this form.  You can pay in cash (please be sure it is in
an envelope with the form) or a check made out to West Elementary PTO.
This should be submitted by the deadline of Friday, September 22nd,
