ADDDITIONAL FLU CLINIC

 

MONDAY, DECEMBER 17, 2007

 

TOWN OFFICES

36 BARTLET STREET, 2ND FLOOR CONFERENCE ROOM

4:00 P.M. - 7:00 P.M.

 

 

1.       This clinic is open to all Andover residents. 

 

2.      Families are welcome to this clinic. There are a limited number
of doses of FluMist, the live nasal vaccine for persons aged 2 to 49
years of age (certain health restrictions apply).

 

3.       Registration forms are available at the Senior Center, Library,
Health Department, the Town Offices and the Andover Town House. Although
pre-registration is not mandatory, filling out the attached Vaccine
Administration Record and bringing it with you the day of the clinic
will make things move quicker.   

 

4.      If you are 65 or over, your Health Insurance Number is being
requested for reimbursement purposes. Simply fill in the number next to
the appropriate healthcare plan on the form.  If covered by Medicare,
please remember to add the letter to the end of your Medicare number. If
you are covered by more than one plan, please list them all. If you are
unsure of your health care coverage or have questions concerning your
Medicare/Healthcare number, call the Health Department at 623-8295 for
assistance. In case there is a question at the clinic, please bring your
healthcare insurance cards with you.  

If you are not 65 or over, you may leave those questions blank.

 

The clinic is free regardless of your health care coverage.
